SOS Program
Senior Outreaching to Seniors Program

When you need some basic help at home, the SOS program can meet your needs, providing you with maintenance and support services that promote your independence at home and in the community.This program provides a planned and managed package of community care for you if you have complex low - level care needs, but can still live in your own home.

Steps in the Program Delivery Process

Step 1: Individual signs a Participant Agreement agreeing to an assessment to determine if services are needed.

Step 2: Individual is assessed by the IHA assessment tool.

Step 3: Using assessment results the executive of the society decides level of care required and approve the most appropriate services.

Step 4: Society advises care recipient by letter of the outcome of the assessment and the next steps.

Step 5: Approved Service Provider (based on available resources) accepts care recipient.

Step 6: The Service Provider develops a Care Plan which imcludes a range of services required to meet the assessed care needs.The Care Plan is attached to the Participant Agreement and submitted to the society.

Step 7: Services are provided, monitored, manage and reviewed by the society.

 


Community Primary Healthcare

  • Productivity -- the cost of services and the quality, type and nature of services for a health problem or care episode.
  • Accessibility -- promptness and ability to visit a primary healthcare position and ease of assessing services.
  • Continuity -- the extent to which services are offered in keeping with the health needs and personal context of participants.
  • Quality -- the total appropriateness of care is perceived by participants or professionals, including compliance with guidelines, as well as the suitability of services.
  • Responsiveness -- consideration of and respect for the expectations and preferences of service providers and participants

We listen to the community's feed back to develop programs that complement our residents' current interest as well as to help them develop new ones.


Volunteers/Donations

  • The involvement of volunteers within the society will be encouraged and supported whenever possible.
  • AWE recognizes that volunteers play a very important role and their efforts contribute highly to the overall sucess of the organization.
  • All volunteers offering their services to the society will have their offers dealt with promptly and they will be given a warm welcome.

  • We are proud of the fact that we supply primary health services to our small, remote community.
  • We are not privately owned and our revenue is obtained solely through government subsidies, fundraising and user fees.
  • We need your support, as through your contributions we will endeavor to provide the very best quality of life for our seniors for many years to come.

Community Programs

AWE works with individuals, families and the community to strengthen their capacity to take more responsibility for their health and well-being. We provide education and advice on helping community members access resources they need from other agencies. The society works together with others on health promotion initiatives within the community. We are working to address conditions that affect health. As such, AWE contributes to the development of a healthy community.

 Our programs are accessible first level services that promote health, prevent disease and provide rehabilitative, supportive and palliative services.
